Transportation
The Pulaski Senior Center has been meeting the transportation needs in their community since September 2001 with the assistance of the American Red Cross transportation department in Green Bay. Since a vehicle has been placed out in Pulaski an estimated 85 clients per month receive 175 rides per month to various medical, nutritional, and social appointments in the Pulaski township and the surrounding cities such as Green Bay, Appleton, Shawano, and Oshkosh.
An 8 passenger 1 wheelchair accessible small bus is used in Pulaski to assist individuals that may need a little more help or need the lift to assist them in their wheelchair.

WHO QUALIFIES 
Individuals 60 years of age or older and/or any individual with a disability qualify for the transportation service. If the individual is under the age of 60, a physician certification
form is required before using the service. This form is available on the website or contact the Pulaski Senior Center at 920-822-8100 for a form to be mailed or faxed to you.
HOW TO REGISTER 
Clients interested in registering for our service will be asked to provide the following information:
- Name
- Address
- Phone
number
- Date of birth
- Are you in a wheelchair?
- Will an escore be riding with you?
- Emergency contact: Name
- Emergency contact: Phone number
The client certification form is available online for you to print off and mail in or call 920-227-4272 to have this form mailed or faxed to you.
COST

Within Pulaski, the cost for a round trip ride is $2.00. For trips outside of Pulaski such as to Green Bay or Appleton is $6.00 round trip. Client must pay the volunteer driver once the ride is provided. For in town trips a 5-ride ticket for $10.00 or a 10-ride ticket for $20.00 can be purchased from the volunteer driver, at the Pulaski Senior Center or the Aging and Disability Resource Center of Brown County. We encourage tickets to be purchased in advance if possible. Any additional funds paid are considered a donation towards the transportation program, which helps to offset the operating costs of fuel and vehicle maintenance.
